Proof of Insurance
Should you need proof of insurance for the venue you are using be it for games or practices or if you are participating in a parade fill in the below form.
Ensure you fill out all the applicable spots; your league name, name and address of venue, email address of who we need to send copy from insurance company.
Please make sure you fill in the correct info and find out if the venue needs people listed on the insurance form as additional to be insured.
